Primary Adenocarcinoma in a Cystic Duct Stump

Abstract

Carcinoma of the bile ducts was first recognized as a distinct clinical and pathological entity by McNeal in 1835. Its occurrence is uncommon, and it comprises a small percentage of all malignancies. Even rarer is primary adenocarcinoma of a cystic duct stump. To our knowledge no instance of this entity has been reported, and we therefore present the following case. <h3>Report of a Case</h3> A 57-year-old white man (35405) was admitted to Maimonides Medical Center Feb 19, 1967, complaining of increasingly severe episodes of sharp right upper-quadrant and epigastric pains of two months' duration. The pain occasionally radiated to the left lower quadrant and was associated with loose bowel movements in which neither blood nor mucus was grossly visible. Six years previously, the patient had had a cholecystectomy. The operative diagnosis was benign cholelithiasis. One year prior to admission the patient had been hospitalized for similar pains.
