This report describes a young patient with miliary tuberculosis whose clinical course was complicated by recurrent and bilateral pneumothoraces. The extreme rarity of pneumothorax complicating miliary tuberculosis raised a question as to the correctness of the diagnosis and led to consideration of lung biopsy. The possible pathogenesis of pneumothorax in miliary tuberculosis is described.
