We related body weight, expressed as a percent of the ideal (%IBW), to the numerous other features of the disease recorded in this data set. Body weight was directly related to FEV1 (p = 0.0001), so that all subsequent analyses of body weight had to first consider FEV1. Mortality appeared to be influenced by body weight independent of FEV1. In patients with %FEV1 less than 35, mortality increased with decreasing body weight (p = 0.093), and this relationship was stronger in patients with %FEV1 35 to 47 (p = 0.048) and even stronger in patients with %FEV1 greater than 47 (p = 0.007). After adjusting for FEV1, body weight was a powerful positive correlate with exercise capacity (p = 0.0001). Body weight was also inversely related to %TLC (p = 0.0408) after adjusting for FEV1. Body weight was a powerful predictor of diffusing capacity (p = 0.0001) in patients with the same FEV1. These results support the hypothesis that factors related to nutritional status are an independent influence on the course of COPD.1e:T427,To assess the consequences to the human diaphragm of alterations in body weight and muscularity, we measured the mass, thickness, area, and length of diaphragm muscle at necropsy. Of 33 subjects who were clinically well until sudden death, 27 had sedentary occupations and normal weight (group N), while 6 were nonobese laborers whose average weight was 40% greater than normal (group M). Among 37 patients dying of more prolonged illness, 23 were of normal weight (group W), while 14 weighed 71% of normal (group U). Subjects with obesity, chronic pulmonary disease, or edema were excluded. Disease per se did not significantly affect diaphragm dimensions. However, in group M diaphragm muscle mass, thickness, area, and length were 165, 129, 125, and 117% of normal (P less than 0.005), whereas in group U the corresponding values were 57, 73, 77, and 83% (P less than 0.001). Thus alterations in body weight and muscularity profoundly affect diaphragm muscle mass, causing a nearly threefold variation between muscular normal subjects and underweight patients.1f:T45b,Total parenteral nutrition (TPN) using glucose as nonprotein calories was associated with increases in O₂consumption (V̇O₂) and CO₂production (V̇CO₂). The magnitude of the changes was a function of the patient's clinical state and glucose load. Depleted patients showed a minimal increase in V̇O₂, while V̇CO₂increased 23%. Minute ventilation (V̇_E) increased 32%. Hypermetabolic patients (major trauma, sepsis) had a 30% increase in VO₂and a 57% increase in V̇CO₂, while V̇_Eincreased 71%. Patients with mild to moderate injuries (energy expenditure ±15% of normal) showed a 21% increase in V̇O₂and a 53% increase in V̇CO₂, while V̇_Eincreased 121%. Large carbohydrate intakes were associated with increases in CO₂production in all patients, while increases in O₂consumption were seen primarily in hypermetabolic patients. These changes suggest that the high glucose loads of TPN may be a physiologic stress. (JAMA243:1444-1447, 1980)
