Transition Planning Evaluation

Transition Planning Evaluation is a systematic assessment framework within special education that identifies and measures the effectiveness of individualized strategies and supports designed to facilitate the movement of students with disabilities from school to post-school activities, distinguishing it from general educational evaluation by its focus on longitudinal, person-centered outcomes in education, employment, and independent living.
