Unfortunately, the new liver becomes reinfected with the virus in nearly all recipients (1). Many patients go on to develop chronic hepatitis B and cirrhosis (2). Because long-standing chronic hepatitis B infection is a risk factor for hepatocellular carcinoma (3, 4),these patients may also be at an increased risk for developing this tumor. We report the first documented case of primary hepatocellular carcinoma developing in the hepatic allograft of a patient with recurrent chronic hepatitis B.
