Medication Record Completeness

Medication Record Completeness is a metric that evaluates the extent to which a patient’s medication record contains all relevant pharmacotherapeutic information, including current and past medications, dosages, frequencies, routes, start and stop dates, and sources of prescription, thereby distinguishing it from partial or fragmented medication documentation within clinical informatics and health data management.
