Long-Term Care is a system of health, personal, and social support services provided over an extended period to individuals with chronic illness, disability, or functional dependency whose ongoing needs for assistance with activities of daily living, supervision, and condition management exceed the scope of acute or episodic care, with the aim of maintaining functional capacity, autonomy, and quality of life across home, community, and institutional settings.
