Abstract

Two patients with pulmonary reactions secondary to nitrofurantoin hypersensitivity are presented. Relationship to the drug was confirmed by rechallenge, permitting observations during four distinct reactions. Serial blood counts, venous pressures, circulation times, chest roentgenograms, electrocardiograms, pulmonary function measurements, and arterial blood studies were made in addition to frequent vital signs and chest examinations. The combination of acute onset and rapid clearing of noncardiac pulmonary edema associated with fever, leukocytosis, and usually eosinophilia distinguishes this reaction from other commonly occurring cardiopulmonary syndromes.
