Completely blocking T<sub>Eff</sub> cell trafficking with vedolizumab, while simultaneously permitting residual homing of powerful T<sub>Reg</sub> cells in an optimal 'therapeutic window' based on target exposure levels might be a strategy to optimise treatment outcomes in patients with IBD.
