Summary Adherence to the time honored principles of accurate reduction, impaction, and rigid fixation in the treatment of displaced fractures of the femoral neck must be emphasized. A significant improvement in the rate of osseous union and a remarkable decrease in the incidence of late segmental collapse of the femoral head can be effected by the muscle transplant operation. The addition of supplemental bone to fill the large defect in the posterior neck of the femur in 70 per cent of our cases accelerates the speed of osseous union and enhances the chances of obtaining union.
