A case is reported of aortico-left ventricular tunnel with surgical correction in an 11-year-old girl. The diagnosis should be suspected when auscultatory and peripheral signs of aortic incompetence are present in an infant with massive cardiomegaly and left ventricular hypertrophy. The angiographic demonstration of a localized diverticulum arising from the aorta above the sinuses of Valsalva and communicating with the left ventricle confirms the diagnosis.
