Treatment of clubfoot requires a detailed knowledge of the underlying pathophysiology. We explain the concept that led us to propose our technique called functional treatment. It is centered on progressive manipulations to correct the different deformations step-by-step. These manipulations are performed according to four stages; treatment is completed by light splints, which are used between sessions. As this technique does not give a complete reduction of the foot, complementary surgical correction becomes necessary to complete the functional therapy. Surgical correction follows the same principles as manipulations: each residual deformity is corrected à la carte.
