Using two case examples drawn from south and southeast Asia and the Newly Independent States in eastern Europe this document examines evidence of environmental influences on rapid HIV spread among injecting drug users (IDU) and the implications for assessment and response in these regions where drug injecting is a pivotal feature of HIV transmission.
