To dilate or to operate? That is the question In this issue Fellows et al report their experience with pneumatic dilatation in the treatment of achalasia of the cardia. Their conclusion that pneumatic dilatation is safe and effective and should be the initial treatment for all patients with achalasia may seem provocative to some surgeons, and warrants comment.
