Abstract

THE TRANSFUSION of ABO and Rh incompatible blood should never occur, yet reactions to so-called compatible blood are being reported with increasing frequency. Such episodes present a diagnostic challenge and raise serious doubts about our understanding of blood group incompatibility. The purpose of this paper is to report a case of delayed hemolytic reaction occurring eight days after the transfusion of the last of eight units of apparently compatible blood. Although antibodies were not detected at the time of the transfusions, three were identified thereafter. <h3>Report of a Case</h3> A 58-year-old white man was admitted to the Evanston Hospital on Sept 9, 1965, because of gastrointestinal hemorrhage. He gave a history of duodenal ulcer of 23 years' duration with episodes of bleeding in 1942 and 1947. He had received 3 units of whole blood in 1947.
