During its brief history, the safety of intraoperative transesophageal echocardiography (TEE) has been attested to by the paucity of reported complications attributed to its use. We present a previously unreported case of a Mallory-Weiss type, distal esophageal tear occuring secondary to TEE monitoring in a patient undergoing aortic valve replacement and coronary artery bypass graft surgery
