Abstract

This volume is a very timely, extremely well-organized, thorough, easily read work that is scholarly in its scope of text and in the long reference list. Several well-known references are not quoted, but this does not in any way detract from the very helpful bibliography contained at the end of each chapter. The table of contents and the sequence of chapters are thoughtful, but, because several topics are covered in multiple chapters, it can be somewhat awkward to look up answers to specific questions. There are certain omissions that are unfortunate but that do not unduly detract from the overall worth of this volume. In chapter 5, in the first table, diabetes mellitus is not specifically listed as an associated condition, although it is discussed subsequently
