Abstract

To the Editor.— Granuloma annulare (GA) and sarcoidosis represent well-described granulomatous disorders of unclear pathogenesis. Although generally considered individually distinct, GA in association with sarcoidosis has been described in three prior reports.1-3We report another case of sarcoidosis that presented with GA. The coexistence of GA and sarcoidosis may reflect a shared underlying mechanism of pathogenesis. Report of a Case.— The patient, a 57-year-old man, was admitted with fever, night sweats, malaise, and arthralgias of 2 weeks' duration. Previously he had been in excellent health. Physical examination was remarkable for tenderness and effusion with overlying erythema involving the right wrist and the ankles bilaterally. Multiple asymptomatic, erythematous papules, some discrete and others coalescing into annular plaques, were present on the shoulders, the thenar eminence, and the dorsal aspect of his hands and feet. The skin lesions blanched with diascopy, and they appeared concurrently with his systemic symptoms. Laboratory evaluation
