Single-photon emission computed tomography imaging of the liver using labeled RBCs is very useful to evaluate the vascularity of hepatic lesions. This case describes a 59-year-old woman with a lesion in her spleen seen on ultrasound, CT, and MRI. Splenic SPECT with labeled RBCs showed the mass to be avascular. Histologic study of the resected spleen showed an avascular mass with a final diagnosis of inflammatory pseudotumor of the spleen.
