The casual answer to this question is that it can't be done. But as a matter of fact, whether we recognize it or not, we all assume that it can. We even go further, and make the assumption that we know how to do it. Accredited hospitals are required to keep certain records attesting to the quality of care. For example, each clinical record is expected to carry the information that the patient was discharged as "RECOVERED," "IMPROVED," "UNIMPROVED" or "DEAD."
