Five children are described with aketotic, hyperosmolar, and non-diabetic hyperglycemia. Four of the children were suffering from heat stroke, the etiology was undetermined in the fifth. The cases of heat stroke were managed by reduction of the elevated temperature and fluid replacement. No insulin was used and the elevated blood sugars returned promptly to normal. A possible mechanism is discussed.
